How the plan works
This is an HMO dental plan. First you
select a dentist by downloading the
Dental Directory and selecting a dentist in your area (jot down the
facility #, which is required on the application). Then each service
provided by a dentist has a set co-pay, depending on the type of service
that is performed. However cleanings, x-rays and other types of
preventative services are at no-charge.

If you are ready to apply, simply
download the application, print it, follow the directions and mail the
application and first month premium back to the address listed in the
directions that come with the application. If you have any questions,
